I am looking for individual health insurance that would cover basic things such as a regular hospital visit, eye exam, dentist, etc in the short run for at least 6 months. As well as cover minor accidents like having a twisted ankle. I'm not really looking for something that would cover me if I had cancer or a major health problem. I'm looking for something with medical, vision, and dental. Pretty much preventive care and maintanence. I am looking to switch jobs since benefits are not offered where I work.

I'm under 25 and living with my parents at the moment. My Mom just switched jobs, and with that we had to transfer to a new health insurance company.
Out of the whole family, I've been denied coverage under the group plan for unspecified "medical reasons" (we've been told that we'll be told what they are if we appeal).
I'm generally a healthy person (with the exception of a minor history with anxiety attacks that I've never needed to take medication or go to counseling for) and my medical history shows that.
So anywhoo, the bottom line is we're appealing the decision. And I wanted to ask for some advice. Have any of you ever appealed a denial of coverage before? What would you recommend doing as preparation?
Basically, me and my friend were playing football Sunday a week and a half ago and my dad threw a pass and me and my friend jumped for it. We slammed into each other in the air (Which is Ok but I'm 5'8 he's 6'2) and I basically started doing a cartwheel motion in midair and landed on my knee with that momentum. I heard a loud crack (Might've sounded more like a pop can't exactly recall.) Tremendous pain came around my knee (No specific area, all over.) I couldn't put any weight on my leg for a few hours, I guess I Might've been in shock or trauma or something?
Anyways doctor told me to get an MRI (I'll explain why I haven't yet later) and gave me Meloxicam for the swelling (very little swelling in my knee.
Anyways my amateur analysis is that I strained the tendons and possible have a minuscule meniscus tear (alliteration ftw). Over the past week and a half My knee has gotten much better and I can walk 90% as normal as before the injury. The only problem, which is what I'm concerned about, Is that I still can't fully extend my knee, I'd say maybe 85% extension. I also can't fully bend my knee, maybe 95% bend capacity.
So my question is Why can I still not fully Bend/Extend my knee without minor pain? (I can bend it fully with some pain, I can extend it to 90% with pain.) This makes climbing stairs a real pain in the ***.
Anyways, the reason I have not gotten an MRI yet is because recently my parents switched Health Insurance plans and we don't have the card yet. Without the card the MRI place is charging $800 some in CASH. My parents refuse to pay that soooooo.... yea.
Swelling has gone down and you can't even tell my knee is injured if you didn't see me walk. I also get pain whenever my knee goes into a lateral motion. Stepped on a pebble the other day My knee slightly twisted (laterally) and extreme pain came.
I dont want any answers saying" OMG OMG OMG GET AN MRI". I know, I just can't atm but I will ASAP.
